What are indications/artifacts of a *poorly* implemented RAG system?

I don’t know about you, but I get a sense that RAG will impact our lives significantly as it is implemented in various forms and modalities over the next few years (it’s relatively simple and cost effective, for the most naive forms of RAG). Video, audio, images, text, and more. Documents, transcripts, lectures, security footage, and more.
