Converting Soft tokens to Hard tokens in Llama2

I'm working on a small project that requires me to work on the token embeddings, make small changes to them and then map the modified embeddings back to a new token. I've done a good bit of research and can't seem to find a concrete answer/way to get this done. Since it's highly dimensional [32000,4096], what are ways I could probably go about this?
