Say we have a book with many chapters, how do you detect plot twists?

I tried with sentiment analysis but it’s not good enough, since the book goes basically like:

Man is about to meet an old friend -> he finds out he died -> friend wakes up from the coma and is alive -> he’s an impostor and the actual friend did die.

Any other ideas?
